§ 99.12 BURIAL PERMITS; REQUIREMENTS AND RECORDS.
   The Cemetery Administrator shall maintain a register of all burial and disinterment permit records received. The permit for the burial or removal of any corpse shall be presented as a written request from an undertaker, physician or some member of the family of the person whose remains are to be interred or removed, showing the decedent's name, age, date of death, county and state of death and whether the decedent was cremated. These records shall be maintained by the Administrator and submitted to the Barren River Area Health Department.
