§ 155.106  APPLICATION FOR A SPECIAL USE PERMIT.
   (A)   An application for a special use permit may be obtained from the Village Clerk. The completed form shall be given to the Building Commissioner, who shall then refer it to the Board of Zoning Appeals.
   (B)   The application shall contain information as prescribed by the Building Commissioner including the following information and material:
      (1)   Name and address of the applicant;
      (2)   Name(s) and address(es) of the owner(s);
      (3)   An accurate sketch drawn approximately to scale of the site and the surrounding area at least 300 feet measured from the boundaries of the property; and
      (4)   A statement of the specific conditions and circumstances which exist and are peculiar to the particular parcel of land in that district.
