§ 90.36  APPLICATION FOR LICENSE.
   Written application for a license shall be made annually by the owner on a form provided by the village. The owner shall be identified by name, address, and telephone number and the dog or cat by sex, age, breed, color, and call-name and the rabies inoculation certificate number, date, and administering veterinarian given.
