§ 110.19 REPORTS.
   (A)   The grantee shall file annually with the Board not later than four months after the end of its fiscal year during which it accepted a franchise hereunder and within four months after the end of each subsequent fiscal year, a copy of its report to its stockholders (if it prepares such a report), an annual summary fiscal year report showing gross revenues received by the grantee from its operations within the county during its preceding fiscal year and a letter containing audited gross revenues data compiled by a CPA showing the gross revenues of its operations within the county during the preceding fiscal year.
   (B)   At the same time of filing the above reports, the grantee shall file a current system map, depicting its actual service area. A current list of all grantee's officers and directors including business addresses and business telephone numbers. The names, addresses and both business and residential phone numbers of the cable television system manager and chief engineer.
