§ 152.056 LOCATION MAPS.
   (A)   The maps used for showing the general location of subdivision and its relationship to surrounding areas shall be at a scale of one inch to 1,000 feet (it is suggested that the township maps obtainable from either the village or county planning office would make a good standard base map).
   (B)   The maps shall be prepared similar to the location map below, in this division (B). The dimensions of the maps shall be adequate to show the true relationship of the proposed subdivision and the village limits.
 
   (C)   The following information shall be shown on the face of the plat:
      (1)   The outline of the proposed subdivision with the area of the subdivision cross-hatched;
      (2)   The name and address of the subdivider;
      (3)   A legal description of the proposed subdivision;
      (4)   A north point and graphic scale;
      (5)   The date of submission;
      (6)   The location of surrounding schools, major streets and highways, shopping and employment centers, and other major facilities;
      (7)   A written statement of how water and sewage treatment will be handled;
      (8)   The existing zoning on the tract to be subdivided and, surrounding the tract; and
      (9)   The ownership of surrounding land area.
