§ 150.017 APPLICATION.
   (A)   (1)   A person seeking issuance of a permit hereunder shall file an application for such permit with the Municipal Clerk-Treasurer. The application shall be in writing upon forms provided by the Municipal Clerk-Treasurer, and shall be filed in the office of the Municipal Clerk-Treasurer.
      (2)   The application shall set forth:
         (a)   A description of the building proposed to be moved, giving street number, construction materials, dimensions, number of rooms and condition of exterior and interior;
         (b)   A legal description of the lot from which the building is to be moved, giving the lot, block and tract number, if located in the city;
         (c)   A legal description of the lot to which such proposed building is being moved, giving lot, block and tract number, if located in the city;
         (d)   The portion of the lot to be occupied by the building when moved;
         (e)   The highways, streets and alleys over, along or across, which the building is proposed to be moved;
         (f)   Proposed moving date and hours; and
         (g)   Any additional information which the Municipal Clerk-Treasurer shall find necessary to a fair determination of whether a permit should be issued.
   (B) The application shall be accompanied by the following documents.
      (1)   Certificate. The owner of the building to be moved shall file with the application sufficient evidence from the County Treasurer that all taxes have been paid thereon, and that there is no unpaid bonded indebtedness existing against the property. The bonded indebtedness shall be determined as an amount that bears the same proportion to the total outstanding bonded indebtedness of the municipality, if the municipality has such bonded indebtedness, as the assessed valuation for the preceding calendar year of the building sought to be removed bears to the total assessed valuation of all taxable property in the municipality for the preceding calendar year, or if the school district has any outstanding bonded indebtedness, an amount which is a similar proportion of the assessed valuation of such building sought to be removed bears to the total outstanding bonded indebtedness of such school district, or both such amounts as the case may be.
      (2)   Certificate of ownership. The applicant, if other than the owner, shall file with the application a written statement or bill of sale signed by the owner, or other sufficient evidence that he or she is entitled to move the building.
      (3)   Liability policy. The applicant shall file with the application a certificate of insurance providing coverage for both personal injury or property damage which might occur during the moving of said building. The minimum amount of coverage allowable shall be $250,000 property damage coverage, and $250,000 personal injury coverage. The City Council may require coverage in greater amounts if it deems it to be necessary.
   (C)   The application shall be accompanied by permit fee in the amount of 10% of the appraised value of the structure being moved if the structure is being moved outside the city limits, which may be refunded by the city if another structure of equal or greater value is placed or erected on the property. Fees so collected shall be credited to the General Fund.
