§ 115.04 APPLICATION.
   An application for a license to sell tobacco, tobacco products, or tobacco related devices hereunder shall be made on a form provided by the city. The application shall contain the full name, address, and telephone number of the applicant; the name of the business at the proposed licensed premises and its address and telephone number; the name, date of birth, residential address, and telephone number of the operating manager or other agent in charge of the premises, if different than the applicant; and any additional information the city deems necessary. Upon receipt of the completed application and license fee, the City Council shall take action on the license application at a regularly scheduled City Council meeting. If the City Clerk determines that an application is incomplete, he or she shall return the application to the applicant with notice of the information necessary to make the application complete.
