§ 51.20 APPLICATION AND PERMITS.
   (A)   No person shall make any type of connection to, repair of or alteration in the water system, sanitary sewer system, or storm water sewer system, except after making an application on a form provided by the city and after receiving a permit issued by the city. The application shall include the legal description of the property to be served, name of owner, the uses for which connection is requested, name of street upon which the property fronts and official street number assigned to the premises as shown by records of the Building Department, and the size of the service line to be used.
   (B)   After the construction, enlargement, alteration or repair of the system is complete the Utilities Division shall be notified. It shall be unlawful to cover any connecting line until an inspection has been completed and the connection and the work incident thereto has been approved by the Utilities Division.
