§ 111.03  LICENSING PROCEDURE.
   The license application referred to § 111.02 shall require only information sufficient to allow the person, firm, corporation or association engaging in activity described in § 111.02, to be contacted by a township citizen or by the township.
   (A)   Information required for the issuance of a license:
      (1)   The name, age and physical description of applicant and each such agent and/or employee;
      (2)   Complete permanent home and local address of applicant and his or her agents or employees;
      (3)   The applicant must submit proof of registration for commonwealth sales/use tax or sufficient demonstration that such registration is not required;
      (4)   Name and address of organization or person for whom the canvass or solicitation is being made, or, if a corporation a copy of articles of incorporation and certification of Federal Income Tax I.D. number;
      (5)   Description of the nature of the canvass or solicitation, or a description of the purpose or causes for which the canvass or solicitation is being made; and
      (6)   The make, model, year, color and license plate number of automobiles used by the applicant, agents and/or employees during the period of canvassing or solicitation within the township and the numbers of their drivers, licenses and state of issuance.
   (B)   Display of license required:
      (1)   Every transient merchant, shall, at all times while engaged in the sales of merchandise in the township, display the license at their location of sales. The license shall be located in a conspicuous location and visible to the public, or other enforcement officers of the township; and
      (2)   Every canvasser or solicitor shall, at all times while engaged in canvassing or soliciting in the township, carry such license upon his or her person, and shall present said license upon request of any resident, or other enforcement officers of the township.
