§ 111.02 LICENSE APPLICATION.

   Application for a license hereunder shall be made in writing to the Village of Anna and shall include:

   (A)   The name of the business applying for a license;

   (B)   The name(s) and signature(s) of the owner(s) of the business; and

   (C)   The address at which the sale, offer to sell, give away, or delivery will take place.
