The application for a permit shall be upon forms to be supplied by the City of Anaheim and shall set forth the following information:
.010 The name and address of the applicant;
.020 Description of vehicle and load to be operated or moved;
.030 Highways over which permit to operate is requested;
.040 Date, hour, duration and number of trips to be made;
.050 Number and location of stops to be made within the City of Anaheim. (Ord. 2389 § 1 (part); May 23, 1967.)
