All applications for licenses shall be made in writing to the license assessor and shall include a copy of the Sales Tax certificate as required in section 3-1-3 of this chapter. The application shall show:
A. The name of the person or persons desiring a license.
B. The name of the business.
C. The kind of license desired, stating the business, calling, trade or profession to be performed, practiced or carried on.
D. The place where such business, calling, trade or profession is to be carried on, giving the street number, if such business, calling, trade or profession is to be carried on in any building or enclosure.
E. The application shall also state any other matter or thing required by ordinance, statute or the license assessor.
F. Any business license application not finalized within ninety (90) days from the date of submission shall expire and a new application shall then be required for that year. The original application fee shall be non-refundable. (Ord. 906, 2-19-2019)
