7-12.01   Customer Notice.
   A franchisee, at the time of initial installation, at least thirty days prior to the time the franchisee changes any customer practice, at least annually and any time upon request shall provide a customer with a written "Notice of a Customer's and Franchisee's Rights and Responsibilities with Respect to the Provision of Cable Service." The Notice shall contain information as determined by the Authority including, but not limited to, a listing of all products and services offered, all rates and charges and conditions of subscription to products and services offered, billing options and practices, complaint procedures, installation and service maintenance policies, instructions on using products and services, customer service office hours and telephone numbers and the address and telephone number of the franchising authority's cable office.
